Prior your appointment:

1. Be sure to call the toll-free number on the back of your health insurance card to find out if your plan includes counseling benefits.
2. If counseling is covered, ask for the following information:
   • number of sessions covered
   • amount of the co-pay
   • amount of the deductible
   • percentage covered for each session
   • authorization number needed for your paperwork
3. After you've completed your forms, please arrive early for your appointment and bring the forms with you.
